Announcing the Patient Care Network!

What is the Patient Care Network? The Patient Care Network is a global chronology of a patient's clinical encounters with physicians and healthcare providers from across the Rochester RHIO region. It is available through RHIO Explore. The Patient Care Network also provides contact information for a patient's care team and how to reach them. Authorized users will have 24/7 access through RHIO Explore to a concise view of the facilities, encounter types, frequency, dates and diagnosis information of a patient. 

Also part of the Patient Care Network suite: MyCommunity Profile. MyCommunity Profile allows clinical providers and users to review and update their professional information in our Patient Care Network (demographics, communication preferences and affiliations).
